Any interaction with the general public poses an inherent risk of exposure to COVID-19. People who show no symptoms can spread COVID-19 if they are infected. COVID-19 is an extremely contagious disease that can lead to severe illness and death. The CDC advises that older adults and people of any age who have underlying medical conditions are especially vulnerable to severe illness from COVID-19. By my attendance at any Christian Student Fellowship, Inc. activity throughout the 2020-2021 school year I voluntarily assume all risks related to exposure to COVID-19. I further understand that despite the best efforts of Christian Student Fellowship, Inc. and/or any employees, contractors, volunteers, and any other individuals involved directly or indirectly with the activities of Christian Student Fellowship, Inc. to comply with the then current guidance by the CDC, such actions will not eliminate the risk of damage, loss, personal injury, or death from COVID-19.

Despite all known and unknown risk including, but not limited to, serious bodily injury, permanent disability, and loss of life, I, hereby expressly and voluntarily remise, release, acquit, satisfy, and forever discharge and agree not to sue Christian Student Fellowship, Inc. and/or any employees, contractors, volunteers, and any other individuals involved directly or indirectly with the activities of Christian Student Fellowship, Inc. and agree to hold said parties harmless of and from any and all manner of actions or omission(s), causes of action, suits, sums of money, controversies, damages, judgments, executions, claims and demands whatsoever, in law or in equity, including, but not limited to, any and all claims which allege negligent acts and/or omissions committed by Christian Student Fellowship, Inc. and/or any employees, contractors, volunteers, and any other individuals involved directly or indirectly with the activities of Christian Student Fellowship, Inc., whether the action arises out of any damage, loss, personal injury, or death to me as a result of contracting Covid-19 while participating in, or as a result of participating in, any of the activities of Christian Student Fellowship, Inc. This Covid-19 Release of Liability is effective and valid regardless of whether the damage, loss, personal injury or death is a result of any negligent act or omission on the part of Christian Student Fellowship, Inc. and/or any employees, contractors, volunteers, and any other individuals involved directly or indirectly with the activities of Christian Student Fellowship, Inc. and as a result you contract or are infected by Covid-19. The Covid-19 Release of liability is effective for each Christian Student Fellowship, Inc. activity I participate in throughout the 2020-2021 school year.

By signing this document, I understand that I may be found by a Court of law to have forever waived my right to maintain any action against Christian Student Fellowship, Inc. and/or any employees, contractors, volunteers, and any other individuals involved directly or indirectly with the activities of Christian Student Fellowship, Inc. on the basis of any claim for damage, loss, personal injury, or death resulting from exposure to or infection from COVID-19 from which I have released those parties herein and that I have assumed all risk of damage, loss, personal injury, or death to myself and agree to indemnify and hold harmless Christian Student Fellowship, Inc. and/or any employees, contractors, volunteers, and any other individuals involved directly or indirectly with the activities of Christian Student Fellowship, Inc. from and against any and all losses, liabilities, claims, obligations, cost, damages and/or expenses whatsoever paid, incurred and/or suffered by Christian Student Fellowship, Inc. and/or any employees, contractors, volunteers, and any other individuals involved directly or indirectly with the activities of Christian Student Fellowship, Inc. as a result of my participation in any of the activities of Christian Student Fellowship, Inc.

I have had a reasonable and sufficient opportunity to read and understand this entire document and consult with legal counsel or have voluntarily waived my right to do so. I knowingly and voluntarily agree to be bound by all terms and conditions set forth herein.
